Some students automatically generalize target sounds. Some take a little deliberate planning! This chart is for that purpose.
Therapy notes: includes goal setting, general information (i.e., the speaking partner, the activity in which the student is self-monitoring), and a 1-5 scale for the student and the speaking partner to rate overall awareness of (target) speech sound production. Also great for involving / informing others of the student's goals and progress, and for progress monitoring. Can be used for the speech setting or for the classroom setting (with some discussion with the teacher!). Not intended to be used with other students.
Materials notes: includes three pages. Page 1 is instruction for the student and for the SLP. Page 2 is a black and white version. Page 3 is a color version.
