This is a really great tool to give out at the beginning of the year. I put it on coloured paper so they can easily access it after I give back each major assignment.
It helps them track their own learning, and actually record the feedback they receive (rather than just look at the mark and move on).
The second page is an extra, that can be used for more observations around midterm. I also like to keep these for parent-teacher interviews, as extra information.
(I collect them before interviews, refer to them, and give them back to the students after)
