This simple checklist is devised by an OTR for OTR's as an RTI method to determine if further evaluation is needed.
Each area is simply defined by a few red flag signs that a sensory diet or sensory support may be beneficial.
Poorly organized sensory systems can affect people in only one sense or in multiple senses.
One person may over-respond to sensation and another might under-respond and show little or no reaction to stimulation or extreme temperatures.
Reviewing this data sheet between classroom teacher and the Occupational Therapist can help decide on helpful strategies and solutions OR if formal evaluation is required.
Consider using this data across school environments asking staff at lunch, recess, specials and even transportation support staff to fill this data sheet out for review by an OTR.
