I've learned a few things after being in the schools for about five years. One of those is things is to KeEp. It. SiMpLe. But, in order to get to simple, sometimes we have to learn what it means to be complicated. I'm here to get you straight to simple as soon as possible. I was given a similar sheet from a fellow service provider to use and thought the format was amazing, however the boxes were much smaller, there wasn't a key and it was very challenging to copy due to the grayscale, so here I am posting a new and improved sheet! I love that you can see the entire year and easily keep track of how often you've seen the child per month to keep up with frequencies. Each child gets his or her own sheet, stick them in a binder and you're good to go for keeping track of all the things! Enjoy and please, please, PLEASE send feedback if you have any! I'm still tweaking the accommodations sheet, however I felt it was worthy of sharing at this point. ;)
