The purpose of this worksheet is to monitor student learning through self-assessment. At the start of the unit, after taking the pre-test, teachers meet with students to review the pre-test and identify growth areas. Teachers identify an item or series of items with similar concepts that were incorrect on the pre-test. This item's score is entered in the 'Pre-Assessment Score.' This development area is entered in the 'Skill I need to work on' and is correlated to the Common Core State Standard(s). The student is responsible for working on the skill at least three times during the unit. They record the activity they did in the 'Activities used to practice the skill' and document how they completed the activity in the bottom section. The teacher can also provide recommended activities for the student to complete as needed. Throughout the unit, teachers can give feedback to students and review progress made based on activities they are completing and the way they justify their thinking. This can be used as formative assessment. After the post-test, teachers meet with students to see the growth made. This form helps teachers monitor student progress and learning, plan instruction, and set differentiated instructional outcomes. Teachers can use this to form instructional groups. It focuses on engaging students in their own learning and creating a sense of ownership. This can also be used as an artifact in teacher evaluation.
