Shape Identification Assessment for Google Forms™ {2D and 3D Shapes}

Shape identification is a core skill that is assessed often by teachers in Kindergarten and First Grade. This assessment, for use with Google Forms™, is designed to make the process of assessing and tracking shape identification simpler and less time-consuming.

The following shapes are included in this assessment:

(With Google Forms, you can easily delete any questions you do not wish to assess.)

2D Shapes

★ Hexagon

★ Octagon

★ Oval

★ Pentagon

★ Rectangle

★ Rhombus

★ Square

★ Trapezoid

★ Triangle

3D Shapes

★ Cone

★ Cylinder

★ Cube

★ Prism (Rectangular Prism)

★ Pyramid (Rectangular Pyramid)

★ Sphere

To use this assessment:

★ Fill in the child's name and the assessment date.

★ Scroll through the form. As the child identifies each shape, simply click correct or incorrect.

★ View results, both for individual students and for the class, immediately!

(This assessment is presented in two sections, 2D shapes and 3D shapes. While assessing, shapes are presented in random order within each section. For ease of use, the reports will show results in alphabetical order within each section.)
