I use this resource when I am working on writing Short Stories with my 6th grade students; they participate in a peer-review once they have completed their initial rough drafts. It's a wonderful tool to help them kick-start their revised drafts the next day. It includes questions to prompt thoughtful reflection and areas to give general feedback; students are also given the opportunity to assess themselves with a CCSS-aligned rubric. It is not genre-specific, and this could easily be modified for use in any middle-grades (5-9) classroom for any fiction piece. A lesson plan is included. Enjoy!
