This is a template for note-taking that is specifically geared towards students who otherwise will copy notes verbatim and struggle with evaluating info for importance. This template is designed for History but can be modified for other contents.
I assigned this method of note-taking to my high school students with Lexile scores less that 1000. I also use a modified version of Cornell notes for my middle group and I teach outlining to my students who are capable of successfully using a more college-style note-taking system.
