Anytime you have parents/families visit your classroom, you want a record of their attendance. This is an easy form for them to fill out on the way in the room and it helps you keep updated information on hand and at-a-glance. Columns include name, parent name, phone #, email, bday, and Y/N to texting (good to know these days!)
I make a folder at the beginning of the year with a stack of these and have them on hand all year. I hole punch the ones that are filled out and carry them in my lesson plan binder. Makes parent contacts a step easier!
