I use this form during our silent reading time. Each student has a day that they meet with me to discuss their book that they are currently working on. I attached 3-5 sheets behind student dividers inside of a thick 3-inch binder. When I meet with a student, I record their book title, reading level, date, what we discuss about their book and set the student up with a new reading goal for the week. We may discuss if they like their book so far, who are the main characters, funniest part, they may read their favorite part out loud, etc. Their reading goals are similar. I may have some students record short summaries at the end of page with sticky notes to share with me, they may have to write facts/opinions that they find in their reading, illustrate a certain part of their book & discuss it, find a "just right" book, etc. This system is a great way to make sure that students are sticking with a book on their individual level. My students LOVE meeting with me! I usually meet with 3-4 students in a small group on the same day so that others can see what other students are reading. They love to read each others' books after discussing them in our group (I call this Book Club). If a child meets his/'her goals for the week, they get a punch on their punch card and one they receive all 10 punches, they receive a day in the loft. Book Club usually takes me 15-20 minutes which works out perfect because everyone else is silent reading during that time! :)
