This is a great end of year portfolio piece or piece of work to send home to show parents more about their child's reading ability!
Students are asked what strategies they use when they get a word in a book they do not know. My students are used to using our 'reading strategy cards' all year and give examples of these and any other things they may do to help them.
Then then read their book which I use from the PM Benchmark set. I write in their current reading level and take a photo of them reading their book. After reading I ask them one thing they did well and one thing they could work for next time.
I enlarge this sheet to A3 and cut out and stick onto coloured card. I put the picture of them reading their book into the space on the sheet and fill in their answers.
