I have a student binder with the name of each of my students on a tab. This is where all the data ends up: conferencing pages, running record charts, etc. In the front of the binder I keep the old weekly planning charts. In my grade book binder I place the classroom observations to make grades easier at the end of the trimester.
Individual Reading Conferences, Individual Writing Conferences, Individual Math Conferences
Make enough copies for each of your students in your class. Write their name on the top of each one. Place on a clipboard. After meeting with your student, place their paper at the back of the pile or if you need to meet with them more frequently after a few other students. Rotate through class list.

Behavior charts, I use data from assessments, observations from lessons, student class work, etc.
For areas where at least 3 students need the same skill I create a group. If it is only 1 or two students I hold an individual conference. First, I write the skill, the students and either a 1, 2 or 3 day sequence of how to address the skill. Each time I meet with the group I jot a note down reminding myself of what the students did well, need more work in or students that can be released from the group or any assignments I gave them.
