This template is designed to use to plan and record interventions with small groups based on individual objective. I use this template to plan for small group intervention based on individual goal or objective. I collect data from benchmarks, state tests, CBA, or classroom assessments. I then make a list of each student that scored below 70% on any objective. I then group the students based on similar objectives. I usually have 2-3 goals per small group.
I list the students for each group under ‘Group Members’. Then I list 1-3 goals across the top in the individual boxes. Once a student has mastered a certain objective I write mastered next to their name. When I focus on the goal a student has mastered, I just do not call them over with the rest of the group.
Each day I record the date in the ‘date’ box. I write down which goal #1-3 that we are working on in the ‘focus goal’ box. I write down what activity I am doing to work on that particular goal in the ‘intervention box’. I write notes about any student’s progress or something else to try in the ‘notes’ box. I list the students’ initials in the assessment box to take attendance of who was in group that day. In the ‘mastery’ box I write either a check for mastered the activity, plus sign for showing progress towards mastery, and a minus sign if the student is not progressing at all.
