The first chart is to be used in small group reading/math to monitor students progress and achievement with regard to a specific standard.
The second chart is to be used for students with IEPs to collect data and monitor progress with regard to specific IEP goals and objectives. Simply type the information from the student's IEP into the appropriate box and print as many copies as you will need to collect data throughout the year. I co-teach with two general education teachers and a para, and I use this tool myself, as well as the teachers I work with to gather data when I am with students, as well as when I cannot be with them. This is a great quick way to gather and record data.
