I like to track student behavior throughout the day. By doing so, trends are easily noticed and plans can be made to help students through specific times/areas of difficulty. It also allows students who may have a rough morning know that it doesn’t have to define the rest of their day.
I have used both the stop light system and the smiley face systems with these tracking forms. Students who follow the classroom rules and are meeting behavioral expectations receive a green/smile. Students who need reminders to stay on task or follow directions receive a yellow/straight face. The yellow/straight face acts as a warning. Students who are unsafe, refuse to complete work, do not follow directions, etc. receive a red/sad face.
I also like to have a place (I copy it right on the back) to write notes as to why the student received the color/smile. This also helps track/monitor when specific behaviors occur. Depending on the student, sometimes I will only keep this sheet for my records and only send the colored side home.
