This snapshot IEP document provides a basic overview of the contents of an IEP in one page. This document is helpful for special education teachers, general education teachers and paraeducators.
As a special education teacher, I print these out blank, fill in, and make a copy for myself and all general educators and paraeducators that work with the students on my caseload. I put my copy in front of the student's IEP in my caseload binder. Other teachers always appreciate having a concise view of the IEP without having to read through all of our special educator lingo.
For the academic info, I usually include the student's MAP scores and grade level correlation. I also include details about the student's home life or outside agencies on the blank page. I've listed most of the supplementary aids and services you will need, but you can add more a the bottom if needed.
