Hello and thank you for looking at Social Language Questionnaires. I designed these questionnaires to include 10 questions for data collection purposes. The questions vary in form (a variety of ‘wh’ questions), as well as linguistic difficulty. Most questionnaires have questions that require students to verbally problem solve, inference and reason (if they don’t, the questionnaires will be updated to reflect those skills). *** I have been working on this product and the questionnaires for six months, so the complexity of questioning has changed slightly to address those higher level thinking.

This is a growing product and I will be adding questionnaires throughout the year, whenever I have completed five more questionnaires. All of the questionnaires were created for black and white printing (if color was used, it is minimal and still can be printed in black and white). They are single page for easy printing and for some questionnaires, a discussion question is provided for which the blank side of the paper can be utilized. Students can either write their responses on the line, use an additional piece of paper to record answers, verbalize them, or in some cases, you can record their responses, however, I find that the one page for printing and maintaining records works best. In some cases, I even send the questionnaires home for parents to complete with their child. I have decided to share and sell the questionnaires once I created 20, but again, this will definitely increase, so be sure to check back on a monthly basis. (Now includes 25)

How I use the questionnaires - You will see that some of the questionnaires you can use as a Do Now, to have your students ask and answer questions verbally, or as a session activity. Depending on my days and weeks, I use them both ways. I additionally use the questionnaires to keep information on my students as far as record keeping for how they responded, or in the questionnaires that are reflective, I keep to assist in writing personal narratives about them, report writing or just simply so I can establish a rapport and get to know them better. You will see that some of the questionnaires were designed for the purpose to get to know students, establish rapport or to assist in obtaining information when writing IEPs. I often have my students fill them out in the group and then I will keep the completed ones in an organized binder to assist in the process of planning for upcoming years in the annual review process. I am finding that I am getting to know my students and have a better rapport with them as I have this recorded information, especially when interacting and meeting with parents. Who doesn’t love when someone really gets to know your child? The completed questionnaires furthermore provide me with information to assist me in wiring narratives about my students or for planning purposes for the annual review process. I have organized the questionnaires currently in four sections; seasonal, holidays, or recognized times of the year, thematic or establishing rapport / learning about students on another level. I often use these for discussion or in a group to start the week off. Again, many questions were designed for the development of expressive language and expansion, but can be used for vocabulary enhancement, improving verbal problem solving and inferencing and / or linguistic processing.
