This observation form is used by students who are observing a speaker in the "fish bowl" seminar. It's simple to use and leads to great discussion after the seminar.
Students who are observing put a tally mark in a box adjacent to the behavior listed. I collect these and review later to see how I can help students do a better job preparing for the seminar. Since it's a basic form it is best used with elementary grades or with students who are just starting out with Socratic Seminars.
