In elementary kids have so many disagreements - which is totally understandable! When a new prospective teacher came and did lunch yard duty with me she was shocked that more kids (third through fifth grade) did not come to me tattling or needing help with a problem. Actually that day NO kids came (which was unusual, I admit!) But one thing I started about a month into school was helping kids to solve their differences on their own if they could. I had a form in the room similar to this one that they would get and complete with our Class Counselor (one of my classroom jobs.) If they could work it out well and I knew it was a legitimate problem that was solved, I would sometimes - not always - reward them with a small candy or Class Dojo points.
I will leave this file in Word if you would like to tailor it to your class, or if you have to adjust it for the double sided printing.
Thank you for looking!
