This is a powerful tool to communicate the learning goals of your classroom revolving around the Math Content Standard REI.C.5/ REI.C.6 / REI.C.7 (this can be used to assess solving systems of equations even if you use different verbage).
With this checklist, teacher, student, parent, and admin can all clearly see where the student is doing well and what areas need extra attention. This provides a focused path for students to meet proficiency!
The first page is your checklist, using a 1-4 scale. Students fill in their highest score, never going backwards! Once a student performs at a "3" or Proficient level twice, or a "4" or Advanced level twice, they can fill in the "StarBurst" and you sign off! That skill or concept can they be checked off as learned in your gradebook!
The second page breaks down what each individual skill and concept refers to and provides a tracker for students to denote what date they took each assessment.
Depending on how differentiated your class is, you can either have students 100% on their own fill in the dates, or if the class takes assessments together, you can post a "class copy" of this checklist for students to get the assessment info.
