I have developed this form to keep track of students' progression in speaking Spanish during the school year. (THIS CAN EASILY BE MODIFIED TO FIT OTHER FOREIGN LANGUAGES, TOO!)
I print one form per student and then three-hole-punch them to keep in a binder. I make notes and observations monthly for every student, assign them a grade based on the rubric and then pass the forms out at the end of every month so they can assess their progress (or lack thereof). I then collect the forms and place them back into the binder so I can make notes for the next month. This is a paper-saving idea.
It's a good tool to encourage the students to consistently monitor themselves and try to communicate in the target language. The monthly feedback gives them a heads-up on where they are ability-wise, and allows them a chance to self-correct for the next month if they are slipping.
The document is in Word format, so you are free to download and make additions and/or adjustments to the rubric as you wish.
