This is a one page template. The first sentence is an example of what students should write about themselves. Everything in bold is what the students should rewrite exactly on the next line (with their own info filled in). I would suggest that you write in your own personal information to let students know more about you.
I used this to GREAT SUCCESS in middle school grades during an All About Me unit. Students spent 5-7 minutes filling in their info (skipping sentences they did not understand). They crumpled up their papers and put them in a bin. (Make sure you keep your classes separate.) Every day for the remainder of the unit, I pulled 2-3 mystery students, describe them without names, and had the students guess who it might be. (BTW, you can also throw in clothing descriptions the day of.) Students were getting input on target structures every day and were all ears.
This could also be a great beginning of the year activity or perfect in the dead of winter when students THINK they know everything about themselves.
