Student Survey

This is a survey for my students to complete at the beginning of the school year. You can have them complete it the first day or before the year even begins (that’s my preference). Once I have my class list, I send each student a letter of introduction, a copy of this survey and a self-addressed stamped envelope to make returning easy.

Parent Survey

This is a survey for my students’ parents to complete at the beginning of the school year. You can have them complete it at Open House or Meet the Teacher night or before the year even begins (that’s my preference). Once I have my class list, I send each family a letter of introduction, a copy of this survey and a self-addressed stamped envelope to make returning easy. (I include it all with the student letter and survey).

Quality IEP Checklist

This is a checklist that I compiled from a variety of sources and experience. Most of the recommendations come from NYS guide to writing IEPs. This list is something that I TRY to review after I write each IEP to ensure that everything needed is included and to remind myself of some good habits for writing.

Team Communication Worksheets

These are my FAVORITE part of this resource. My district starting using team communication worksheets about 6 years ago and it was life changing – literally. So often it is easy to miss out on meeting with each team member. As much as I try, I never seem to be able to make it around to everyone. Either people aren’t available when I am or I forget to ask something. This sheet has been an amazing tool for me to make sure I have feedback from everyone working with my students. I send this to anyone in our district that works with my students (classroom teachers, teaching assistants, special area teachers, counselors, nurse if appropriate, principal if appropriate, etc.). I do not distribute this to OT, PT, Speech, MPE, or other related services since they enter their own feedback directly into our IEP software.

This tool has been such an amazing way for other team members to communicate and share their thoughts for. I hope you love it as much as I do!

Just an FYI…not every team member loves more paperwork to complete…I make sure to praise and thank everyone who does return it and it really encourages them to complete it. 

Team Communication Worksheets –
Parent Version

Parents are a crucial part of the team! This template is designed for their input. I have also included a sheet to send home with it to provide some directions.

Meeting RSVP

So many of my parents are intimidated by the look of the paperwork from our CSE office that they barely give it a look. That’s why I started sending this home. It’s a simplified reminder of the meeting’s date and time and gives me a heads up on their attendance for the meeting. I send this home about a week in advance of the meeting.

IEP at a Glance

You can handwrite on here or add a text box to insert the information. This is a great cheat sheet of sorts to share with team members and have on hand.

Meeting Notes

Here’s a few different options for meeting notes that I find helpful.

CSE Dates

This goes right at the front of my plan book. I fill in all CSE dates right away, so that no meetings can sneak up on me. I have included two versions for different starting months.

Data Tracking Sheets

I use the Running Record sheet to track student progress as I complete each running record. Helps summarize all of their data in one place.

I my classroom, we take daily fact tests and I track them on the Math Fact Fluency Sheet. It is something I share with students as well to help motivate them to make further progress.

I create a Goal Data sheet for each goal for each of my students and use it to summarize all information I have gathered.

The assessment data sheets can be used to track assessment scores and other anecdotal data. You can track by assessment or by student.

CSE Services

This sheet helps me manage all my students’ services. I can make sure that all services are being provided and at a glance know who needs what. I also print another copy to use for projections and another to mark actual placements for the following year to share with my department members.
