It is important to track how much support a student requires from the paraprofessional that is assigned to work with them. The ultimate goal is to reduce paraprofessional support by fostering increased independence. This form is for the paraprofessional to fill out when working with a student to help give the special education teacher a good idea of how much support is required for specific tasks.
