September 2015 Update: I fully updated the template to make the service categories and locations fully customizable while still maintaining the ability to automatically calculate LRE and service times.
The preview does not do this template justice. This is a powerful yet incredibly simple tool that will simplify the endless paperwork demands of special education.
This template allows you to enter a students daily schedule then it automatically calculates the service time for each service type (e.g., Social Skills, Adaptive Skills, Physical Therapy, Writing, etc.) and by location (e.g., general education, special education).
You enter the start time and end time and the minutes are automatically calculated for you! Choose the service category and location from the drop down menus. The template automatically calculates the totals and color codes the schedule.

This schedules are great to give to the classroom aides to help keep everyone on schedule. I also keep a copy in my substitute binder so there are no questions about individual student schedules.
These are also my parent communication tool. I send home a copy each day so parents are informed of the daily happenings of their child.
How I use this in my room:
-After entering the students schedule I print out the daily forms and weekly summary.
-I use the forms to keep track of the schedules of each student (they can get crazy!) and take notes about goal progress, concerns, and questions throughout the day.
-At the end of the day I keep a copy for myself and send a copy home for the parents. This serves as my data collection and parent communication log (2 birds with one stone!)
-When it is time to update the students IEP I have all of the information I need for the service matrix in one place. I do not have to comb through the students schedule and calculate how much time is spent each day on each goal area.
-If a student's schedule changes I simply make the change on the computer and print out the updated schedules.
In the special education world accountability and record keeping are mandatory. This spreadsheet makes easy work of a monumental task.
This file also includes a sample schedule and directions to make using it effortless.
