Oftentimes, when I was teaching special education, students would get sent to my room for additional support, supplemental instruction, accommodations, or other supports. This sheet is where I would record the students that worked with me during that time and the supports offered to them. It was simply a recordkeeping mechanism if I was asked at a later time how I assisted. It also works well for co-teaching if the special education teacher works with a specific group of students. Additionally, this chart can be used if you assist within a general education setting.
