This is a great way to support engagement of teachers and classroom staff in the therapeutic process, and ensure that therapy services are meeting the student's needs in the classroom. This form also encourages the development of relevant, classroom-based goals.
To ensure valid and reliable data collection, the documentation form includes data on day, activity, time spent on the activity, and recorder's initials. Use of a rubric is suggested to maintain objective data on progress.
This download includes: a cover page, 2 pages of documentation in a vertical log, 2 pages of documentation in a more concise format, and a page with an intelligibility rubric and instructions for the person responsible for completing the form.
