These checklists are designed to make your speech-language evaluation process easier and more organized! Save time with these handy forms, which make report writing and collecting valuable input from parents and teachers more streamlined and simple. They can be used during the evaluation process, as well as preparing for yearly IEP meetings. These are appropriate for any grade level, but especially the elementary level.
The first page is a checklist to keep track of what you have done when during the evaluation process, and what your deadlines are. This is very handy to keep in the front of your student’s file. The next two pages are designed for you to take quick notes on your student’s performance during your evaluation sessions. (I find this information very helpful when writing the “informal observations” sections of my reports.)
The final two pages are checklists for parents and teachers to fill out to assess what they see, and are written in teacher- and parent-friendly terms. This helps give you an idea of what impact the student’s communication disorder may have outside the therapy room. Also included are two pages of directions and examples.
