When conducting speech-language evaluations with my students, I always like to include a thorough statement about the testing conditions under which the student worked, their demeanor during the testing session (s), and include a general statement about conversational skills. I also sometimes need a visual reminder to write down my start and end times of testing sessions.
As such, I created this checklist as a quick way to note the following information about a student during assessments:
Compliance - How cooperative was the student towards testing?
Affect & Performance - Was the student talkative, sullen, impulsive; did the student require frequent repetition of directions, have a demeanor change during testing, etc?
Appearance & Interview - Did the student appear well-dressed or unkempt? Could the student verbalize personal information, such as birthdate, parent names, or favorite activities?
Number & Length of Sessions - When and during what time (s) was the student tested? Why was testing discontinued?
Environmental Conditions - Were lighting, noise, and temperature levels appropriate for testing?
It is my hope that this checklist will assist the SLP in created a detailed and descriptive behavioral statement for any evaluation report.
