This is a speech language therapy lesson plan template that is functional and easy to use, and incorporates many of the requirements that school districts have for lesson planning.
There is an area to type or write in each students' goals (I add them in short hand), so you won't ever have to look back at their IEP when planning. You can also highlight or bold type which goals you are focusing on for that session if you want.
There are areas to check which curriculum/standard tie-ins you are incorporating, as well as what best practices you are using. These can be changed to fit your district's best practices, or your state's standards.
There is, of course, an area to type or write your planned activity, as well as space to take data during your lesson.
This lesson plan template has been very helpful for me as school districts increase scrutiny of everyone's plans and state standard incorporation; yet, it isn't very time-consuming to use. I hope it is helpful for you, too!
