This is a very quick and comprehensive articulation and language screening tool for 2 year olds through 12th grade.

Plus, you have an articulation screening instrument for all phonemes, including blend sounds on the same form! This assessment only takes a few minutes to administer!
The student visuals for the articulation portion are in black and white or color. You can choose to show the students the visuals with or without words. I have laminated the color visual form that I use, so that they will last a long time. You could also hand out the black and white form and have the student stamp or color each picture as they say it. The target phonemes are in italics and underlined on the therapist form. All phonemes and BLENDS are included.
A form is included so that you can document the students that need a speech screening and it allows you to document if you tried to pull them, but couldn't.
I have also included a *Error Sound Inventory* so that you can document what sounds the student has misarticulated and look at the age of acquisition chart on the form to see in what order you should choose sounds and write your goals.
I usually recommend further assessment, if they score an 80 or below (on the language portion), but use your clinical judgement. As far as the articulation portion, I use the error sound inventory and compare the sounds that they misarticulated to the sounds that they should have at their age, to determine if further assessment is needed.
