This articulation therapy data sheet is easy to use and is very motivating for students. There is a place to write the student's name, the year, the month and day, and the target. Mark in the spaces in each row whether the production is correct or incorrect using a colored marker. If all of the productions in a row are correct, fill in the smiley face at the end of the row. Each row has twenty spaces and the sheet consists of two columns of twenty rows, enough for 400 trials!
Calculating the number of correct productions of trials is easy with the simplicity of this form and you can graphically see the changes in the data over time. Students and parents enjoy seeing all of the hard work!
This data sheet and therapy approach are yielding amazing results! My students watch the data very closely and take ownership of their therapy. They try very hard to get those smiley faces. I reward them with a sticker on a sticker chart for every three smiley faces (see my sticker charts on teacherspayteachers.com) and then they receive a treasure from the treasure chest for achieving a certain number of stickers.
This file contains two pages. The first page is the blank data sheet for you to print and fill out during your therapy and the second page is an example and contains the directions for use.
