Use one sheet of paper for an entire week of planning and data collection!
I can't handles piles and piles of paperwork! I wanted to streamline weekly planning and data collection, so I created this Excel file! It contains 2 spreadsheets which, when printed, leaves you with ONE front and back sheet to plan, collect data, and make notes about what's happening that week! I see kiddos on either a Monday/Wednesday or Tuesday/Thursday schedule, so it was easiest for me to have M/W kids on one side of the sheet, and T/Th kids on the other.
Each Friday, I print a sheet for the following week, and determine which goals I will work on. The student's goals are on the sheet, so there's no flipping back and forth or trying to remember what you need to work on! I might also make notes of any materials/crafts/games I might use. I note any meetings or school events which might affect my therapy schedule, as well as jot down any to-do items.
The font I used is "A Little Sunshine," if you are using a different font, you may need to resize so that everything fits! You may also need to add/remove boxes to customize the spreadsheets for your schedule and students!
