As a new SLP in the schools, I had a hard time keeping track of every component of initial and triennial evaluations that I needed to accomplish. I made this editable, printable half-page checklist that outlines each step of a speech-language assessment at my school sites in the California school system so I could devote my brain power to more important things, like lesson planning.
Just print it out, make copies, and cut on the dotted line. If I'm not the case manager for a particular initial or triennial, I simply cross off anything that isn't my responsibility, like obtaining the school nurse report or sending the meeting notice. As I fill them out, I pin them up on my bulletin board or on my magnetic white board in the order that the assessments are due. This makes it easy to see what I need to get done and to prioritize my testing at a glance.
