The Speech and Language Assessment Checklist is an essential document for IEP meetings, parent-teacher conferences, and student service team meetings. This document is a handy resource to highlight areas of strength and weakness for students who receive speech and language services.
The Checklist is divided into 7 categories:

Prior to meetings, complete the checklist using a +/- to indicate skills the student has, as well as skills that the student still needs to acquire. I like to copy this form in a different color for each quarter. This makes it easy to show progress and present a more complete picture of students. A Comments section at the end, allows for additional information to be included.
