Use these simple parent interviews to collect valuable, detailed information about your students or clients with speech and language concerns or delays/disorders. I created these parent interviews to help SLPs in our district to help determine impact for the little ones we treat and those we are evaluating. We have found that using these interviews, as opposed to interviews with simple check boxes, has helped us to obtain more specific, thoughtful information from parents. Use your professional judgement to determine IF and HOW speech deficits are impacting your preschoolers at home and in the community based on their parent's answers. Also, these are helpful when developing IEPs, writing meaningful goals or objectives for therapy, and making evaluation decisions.

I start by giving the parent the general communication interview. Based on answers, I then have parent(s) fill out the more specific interview for language, fluency or voice if needed.
