Use these simple teacher interview to collect valuable, detailed information about your students with voice disorders. I created this to help SLPs in our district to help determine educational impact for students we treat and those we are evaluating. We have found that using this format for interviews, as opposed to interviews with simple check boxes, has helped us to obtain more specific, thoughtful information from teachers.
I thought you may be able to use this in your practice, too! Use your professional judgement to determine IF and HOW the voice differences are impacting your student at school based on his/her teacher(s)' answers. Also, it's ideal for helping to develop IEPs, to plan targets in therapy, and to make evaluation and re-evaluation decisions.
