Use these simple teacher interviews to collect valuable, detailed information about your students with speech and language concerns or delays/disorders. I created these teacher interviews to help SLPs in our district to help determine educational impact for students we treat and those we are evaluating. We have found that using these interviews, as opposed to interviews with simple check boxes, has helped us to obtain more specific, thoughtful information from teachers.
I thought you may be able to use them in your practice, too! Use your professional judgement to determine IF and HOW speech deficits are impacting your students at school based on their teacher(s)' answers. Also, it's ideal for helping to develop IEPs, to plan targets in therapy, and to make evaluation and re-evaluation decisions.
You will get:
► 3 teacher interviews for articulation (for varying grade levels)
► 4 teacher interviews for language (for varying grade levels) *includes pragmatics
► 3 teacher interviews for fluency (for varying grade levels)
► 1 teacher interview for voice that covers all grade levels

I hope these help to paint a clear picture of your students’ struggles so that you can make an effective plan to help them! If you use them, and you love them, I would love to hear from you. Your encouragement is what keeps me creating!
