I have tried using several different data organization methods for my speech/language therapy sessions, and this data sheet has been the most practical and efficient way I have found to keep data for my caseload.
The sheet is an editable Word document so you can title it with the name of your school or clinic and edit the calendar to include your school holidays. If you want to use this for future school years, simply adjust the dates on the calendar to fit with the new year.
How I use the data sheet: Each of my students has a therapy folder in a bin, and I teach them to sign in independently by finding their folder, circling the date on the calendar, and if they can, writing the date and "time in." When the pages are full, I print additional copies of the second page and staple them together, continuing to use the calendar on the first page.
It is especially handy to be able to "at a glance" see the number of sessions attended versus absences, as well as see data percentages in the same column. This simplifies writing progress reports - A LOT.
