This form was created and implemented in a middle school, inclusive, and co-taught setting. It was used as a method to ensure that students were putting their best effort into their first assessment attempt. In addition, it was a way of having students reflect on their studying habits and notifying parents of what is going on in the classroom. Also, this form keeps students responsible for scheduling a retake time with a teacher.
This form is also aligned to standards based grading. Students indicate what they scored based on a standards based grading system, and how they can improve their score/ understanding of the standard being assessed.
