This is a formative assessment I made based on Gretchen Owocki's fantastic book, The RTI Daily Planning Book (Heinemann, 2010). In order to determine student thinking during reading, students will fill out a Stop and Jot card. Using the tracking sheets I created, sort the cards and determine the next steps for instruction based on actual data, not just a guess as to student thinking! Owocki included descriptions of proficient use of the strategies for you to compare your students' work, and some possible guiding questions during reading. I created one blank tracking sheet for other skills or strategies. I use this during read aloud to gauge the whole class, but the questions are great for small group and individuals as well. Target your instruction to the students at their levels!
