The following product was designed to provide an individualized accommodation track sheet for student and teacher use.
In my classroom, I provided each student with copy of the attached “Student Accommodation Chart” and place a checkmark along the left-side column of accommodation that applies to an individual student. Next, I write their name in the top right hand corner and laminated the sheets (alternatively you may use page protectors) for durability.
When completing classwork, assessments, or other activities, students are encouraged to display their sheet as a reminder of the services available to them. The accommodation chart also serves as a benefit to an instructional staff member not familiar with the student’s accommodations (i.e., resource teacher, general educator, testing proctor). Students are easily able to transport this chart in a folder from room to room.
The “Student Accommodation Chart” provides four columns to checkmark the appropriate accommodation, list the accommodation, provide a visual graphic of the accommodation for young/low-readers and a short description of the accommodation.
