This is a wonderful form to allow your students to complete at a student / teacher conference & then share it with the parents during your conference with them. Completing this made my kids feel so accomplished. This particular report allows them to color in the box next to their reading level; either approaching, on-level, or beyond and explains what their goal is in order to ensure future growth. There is also a section where the student colors in the box based on which particular reading level they are curently reading on from A-Z & it shows which grade level that child is reading on as well. Reading fluency and accuracy are recorded on the bottom of the first page and in bold and underlined I have what a 1st grader should be able to read at this time of the year. The second page has all nine math practices and the student gets to color in the boxes of the practices they can do all of the time. I have my students color in half of the box if they do that math practice "some of the time" and they do not color in the box at all if they "never" perform that particular math practice. There is even a section for student responsibilities for learning! Accountability!!!
