This product from The Student Support Shop contains two forms. One is an initial Student Assessment outline and one is a Family Needs Assessment outline. I like to have easy access to the important information about a student and so I created these one-page documents where I can make short notes as I first meet with a student or family. They are perfect for a school social worker or school counselor who are trying to keep track of 100s of students coming and going.
I copy these documents on colored paper to make them easy to find in my student files and tape them to the inside cover of the file for easy reference. I can then easily glance at the notes when meeting with a student again to make sure I follow-up on important issues and that I remember important details.
Each document has room to draw a small family genogram, make notes about other professionals involved in the student's life (therapist or case manager, etc.) as well as note any referrals you make. It can easily be modified to fit your personal assessment needs and style.
The Family Needs Assessment was created for my work with families with a special needs child. However, much of it is relevant to all families with any unmet needs.
