This is a week long behavior plan for the student, teacher, and parent to stay on track of his/her current learning and behavior. This gives the student the responsibility to rate him/herself on their actions in each subject throughout the school day. Following their own rating on a scale of 1 to 3, the teacher of that subject agrees or adjusts the rating based on their own observations. The parent is to initial the page each night. This is an intervention technique to give responsibility to the student for his/her own learning and actions.
