This is a Student Behavior Plan that consists of 3 different forms: 1) Student Behavior Plan 2) Memo from the Teacher 3) Daily Behavior Chart.
Student Behavior Plan:
This is a 2-page form used to help identify the behavior problem and ways to correct the behavior. It has a checklist of various disruptive behaviors to help recognize the problem and then create a plant to modify the behavior.
Memo from the Teacher:
This is a 1-page form used to inform the parent(s) of why the student was kept in for recess. The teacher fills out the top half of the form informing the parents of the reason for being kept in for recess. The bottom half of the form is where the student will write a letter to his/her parents informing them of what happened. This allows the child to recognize the problem and take responsibility for his/her action. The form is to be taken home, signed by the parents, and then returned to school.
Daily Behavior Chart:
This is a 1-page form used to monitor a child’s behavior daily in 30 minute intervals. One sheet is for a whole week. I would recommend using a folder (either manila or pronged folder). The student should have it on his/her desk or somewhere easily accessible to both you and the student. At the end of 30 minutes, you will make the box with a choice of 3 faces. The student takes home the folder and gets it signed by his/her parents.
