This free 4 page PDF document is a Student Behavior Pre and Post- Survey. The Pre- Survey should be given out to caregivers at the beginning of the school year. This survey asks caregivers to rate how often they see certain behaviors in their child (Always, Often, Sometimes, Never). Survey questions are designed to give teachers insight into each child's behavior. This is helpful because it shows families and administration you are proactive in identifying and correcting behavior that can affect learning.
I adapted this survey from Measuring Social Competence in Preschool-Aged Children Through the Examination of Play Behaviors by Lee (Graduate Thesis, 2006). The survey I created, inspired by Lees, asks parents to rate child’s behavior in categories such as sharing, taking turns, staying on task and aggression.
The Post Survey can be used if you are using mindfulness in your classroom. This Survey ask parents if they notice a positive change in their child's behavior after using mindful techniques in the classroom.
