You can use this sheet to record information on your child’s behavior whether it be good or bad. Keeping records of their behavior will serve as a tool to help spot patterns and will ensure clear communication between parents and administration. This will also diminish negative behavior by getting parents involved so that students will be accountable for their choices and behavior in the classroom.
I have provided a record for teachers to jot down the date, time of incident, section where teachers indicate the positive/negative behavior and how the teacher contacted the parent.
The last page is a contract/Daily Log paper/agreement between student, teacher/parent showing which specific behavior needs to be improved. And the times during the day where they need to exhibit such behavior.. I simplified the faces by allowing the teachers to add in the type of smiley they choose. (happy, sad, or so-so). Then at the bottom there is a section for signatures from student, parent, and teacher..
