This is a dichotomous rubric for students to rate their behavior.
My students complete this form (as a "ticket out") each time that they have a substitute. This gives each student a voice and personal accountability for their behavior. It also provides me with data for when I return to class.
While I am in the classroom, I have students complete this reflection tool as the need arises.
