This conduct chart is used to keep track of student behavior. It's low-hassle and effective; kids really don't like writing in their demerits, and it's a great way to document for parent conferences or behavior referral paperwork.
In its current form the tracker is set up in nine week increments, but because it's in a Word document it's simple to alter the dates and time period.
Students fill in their demerits on the chart corresponding to the day of the infraction using the codes at the top. These codes include "HP" for horseplaying, "CO" for calling out, and 11 others.
The second page of the document is for a weekly conduct and academic grade and a quick note home to parents. There is a space each week for parents to sign and write back, as well.
